Sorry for the delay...  This is a tidbit I had in my PET FAQ, hope it helps:

  Disk Problems:
  - If the disk drive flashes a number of flases repeatedly it is reporting
    an internal hardware fault, here is a chart:

    flashes:         4040                             8050
       #   cause:      location:            cause       location
       1  - Zero Page - 6532, C1, E1         Zero Page - 6532, C1, E1
       2  - ROM       - H1                   ROM       - 2364, L1
       3  - ROM       - L1                   ROM       - 2364, H1

Since 5 seems to be zero page on both the 4040 and 8050 it might be so on an
8250/1001 as well...
